A devastated widow expresses her enduring grief over the loss of her husband in a tragic forklift accident. The incident claimed the life of delivery driver Chris Keegan while he was on duty at Hessle Plant Ltd’s main depot in Castleford, West Yorkshire.

Following the fatal incident on November 20, 2023, the employer has been fined over £400,000 and directed to cover costs totaling £8,000, along with a £2,000 victim surcharge. The Leeds Magistrates’ Court learned that Keegan was involved in returning a forklift truck to a Sheffield site after repairs.

During the operation, as he attempted to reverse the vehicle onto a trailer early in the morning, the forklift slipped off the trailer bed, causing Keegan to be ejected from the seat and trapped between the forklift and a nearby trailer. Despite efforts by emergency services at the scene, Keegan succumbed to his injuries.

In a heartfelt statement, Chris’ wife Dianne described him as a compassionate and selfless individual, emphasizing the profound impact of his tragic passing on her life. An investigation by the Health and Safety Executive (HSE) revealed that the forklift involved had not undergone a comprehensive safety inspection.

Furthermore, it was discovered that the forklift had undisclosed defects that should have been addressed before operation. The investigation highlighted a lapse in safety protocols at Hessle Plant Ltd, where seatbelt usage among forklift operators was not adequately monitored.

Hessle Plant Ltd pleaded guilty to breaching Health and Safety regulations, resulting in a substantial fine of £433,550, in addition to the costs and victim surcharge. HSE inspector David Beaton emphasized the preventable nature of the tragedy, pointing out the critical importance of maintaining equipment and enforcing safety measures in such work environments.
